SUBSCRIPTION AGREEMENT
(pricing for digitizing and subscription in this agreement is for the pre-launch window 2024)
The subscriber (signee) is entering into a personalized streaming service agreement with ARIZONA MEDIA SERVICE LLC (AMS)® d.b.a. as www.QRVideoVault.com® and/or www.QRStreaming.com® to safely store family videos, slideshows, pictures, audio material or documents in a Video Vault on AMS’ online servers. The Video Vault can be accessed and the content can be played back on a camera equipped smart device (e.g. phone, tablet, wrist watch) by scanning a specifically formulated QR-code 24/7 from anywhere in the world. This streaming service is similar to NETFLIX© and HULU© but is personalized for the subscriber’s family memories from digitized video and film material.
Because AMS is striving to keep the QR VIDEO VAULT as secure as possible, there will be no self-upload option for the subscriber and any and all footage or material has to be processed through AMS’ technical team. This fail-safe system was put in place to keep access to the vault extremely secure and all uploaded material compliant with current laws. During this subscription, the subscriber will benefit from 25%-50% discounts for additional video and picture digitizing needs from AMS' regular digitizing and upload services.
Depending on the chosen tier, the digitizing and upload for the subscriber’s first, second or third item* into the QR VIDEO VAULT is FREE of charge. Unlimited additional video or film material can be digitized and uploaded for subscribers at substantial discounts as outlined in tier 1-3 below. Discounted pricing for additional material is valid for the length of the subscriber’s active subscription without limitations and at any time.
By scanning the QR code, the subscriber will be directed to his/her QR VIDEO VAULT with an index of titles stored in the vault. Tap on any title in the vault directory and enjoy your family’s videos immediately on your scanning device.
The user’s subscription to the QR VIDEO VAULT will renew for the selected running time at the end of the subscription period and the same method of payment will be applied to the renewal. Possible future price changes will be communicated to the subscriber via e-mail, SMS/texting or phone call to the data provided. It is the user’s obligation to notify ARIZONA MEDIA SERVICE or the QR VIDEO VAULT if any of the billing information or contact data has changed, for instance if the credit card has expired or the credit card number and/or CCV have changed. If a QR VIDEO VAULT cannot be billed and/or has a delinquent balance, it will result in a frozen QR code. It does NOT indicate that the content of the QR VIDEO VAULT has been deleted, it merely means the content cannot be accessed. Delinquent accounts and frozen QR-codes will be reactivated as soon as payment is received. If no communication can be established between user and the QR VIDEO VAULT or if no payment is received within six months of expiration, the content will be deleted. However – the content can be re-loaded onto the QR code from the stone layer disc back up system at AMS headquarters, for an additional fee of $50.00 per GB, upon the subscriber’s request.

All digitizing occurs in real time without any alteration or editing. AMS cannot be held responsible for any video tape tracking issues, blank frames, prior editing errors or any other distortions or malfunctions on supplied material and/or original recorded footage. All sales are final and because of its business model, no refunds can be issued for early QR Video Vault terminations.
