This form is used to initiate a call back to schedule our FREE local pick-up within a 25 mile radius from Central Avenue and Camelback Street in Phoenix. Should you live beyond the 25 mile radius FREE area, we will still pick up and deliver your order, but in all fairness, we would have to apply a surcharge of $10.00. If you prefer to ship your material to AMS, you can print this form, including your signed subscription agreement and enclose it in your shipment.
Should you decide on shipping your memories to us, you can either hit submit and then send us first this form and later your material via USPS, UPS or FedEx, so we can match your package with your data upon arrival, or you can include a printout of this form in your shipment. Either way: AMS will pay for return shipping expenses of your material and your new DVD(s) back to you after the job is done!
